By creating an account or using MyDotPhrases, you agree to these terms. If you do not agree, please do not use the service.
1. Description of Service
MyDotPhrases is a web-based tool that allows healthcare professionals to create, organize, and access clinical documentation templates ("dot phrases"). The service includes template creation, smart variable substitution, folder organization, team sharing, and encounter template features.
2. Not Medical Advice
MyDotPhrases is a documentation tool, not a clinical decision-support system. The templates provided (including all starter templates) are documentation frameworks only. They do not constitute medical advice, diagnosis, or treatment recommendations.
You are solely responsible for the clinical accuracy and appropriateness of any documentation you produce using MyDotPhrases. Always exercise independent clinical judgment when using any template.
3. Account Responsibilities
- You must be a licensed healthcare professional or healthcare trainee to use MyDotPhrases.
- You are responsible for maintaining the security of your account credentials.
- You must not share your account with others. Each user should have their own account.
- You must not use MyDotPhrases to store protected health information (PHI), patient names, medical record numbers, or other identifying patient data in your phrase templates.
4. Prohibited Content
You must not store in MyDotPhrases:
- Protected health information (PHI) or personally identifiable patient data.
- Content that is defamatory, obscene, or illegal.
- Content that infringes on the intellectual property rights of others.
- Credentials, passwords, or access tokens for other systems.
5. Your Content
You retain ownership of all dot phrases, templates, and other content you create in MyDotPhrases. We do not claim any intellectual property rights over your content.
When you share phrases with a team, you grant team members a non-exclusive license to view and copy those shared phrases. You can revoke sharing at any time.
6. Starter Templates
MyDotPhrases provides pre-built clinical documentation templates when you create an account. These templates are provided as-is for convenience. You are responsible for reviewing, customizing, and verifying the clinical accuracy of any starter template before using it in patient care.
7. Data & Privacy
Your use of MyDotPhrases is also governed by our Privacy Policy. Key points:
- We do not store PHI.
- We do not sell your data.
- You can export or delete your data at any time.
8. Service Availability
We strive to maintain high availability but do not guarantee uninterrupted access to MyDotPhrases. The service is provided "as is" without warranties of any kind, express or implied.
We may modify, suspend, or discontinue any part of the service at any time. We will provide reasonable notice for material changes.
9. Limitation of Liability
To the maximum extent permitted by law, MyDotPhrases and its creators shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of the service, including but not limited to:
- Clinical errors resulting from the use of documentation templates.
- Loss of data due to service interruption.
- Unauthorized access to your account.
10. Indemnification
You agree to indemnify and hold harmless MyDotPhrases and its creators from any claims, damages, or expenses arising from your use of the service, your content, or your violation of these terms.
11. Account Termination
You may delete your account at any time from the settings page. We may terminate or suspend accounts that violate these terms.
Upon account deletion, all your data (phrases, folders, templates, team memberships) will be permanently deleted.
12. Changes to Terms
We may update these terms from time to time. We will notify registered users of material changes via email. Continued use ofMyDotPhrases after changes constitutes acceptance of the updated terms.
13. Governing Law
These terms are governed by the laws of the State of Colorado, United States.
